Over the past decade, many new epilepsy treatments have been approved in the United States, promising better quality of life for many with epilepsy. However, clinicians must now choose among a growing number of treatment options and possible combinations. Unfortunately, randomized clinical trials rarely compare active therapies, making choices between different treatments difficult. Based on a survey of 45 experts on the treatment of epilepsy, these Expert Consensus Guidelines™ address a number of key questions concerning the treatment of epilepsy that are not adequately answered by the literature. Guidelines are presented for three epilepsy syndromes (idiopathic generalized epilepsy, symptomatic localization-related epilepsy, and symptomatic generalized epilepsy) and for status epilepticus. For each epilepsy syndrome and for status epilepticus, treatment algorithms are provided that outline overall strategies; guidelines also cover selection of specific medications and the use of combination therapies. Recommendations are also included on special issues such as the treatment of a woman in her reproductive years who is not pregnant and trying to conceive, of a woman who is pregnant or breastfeeding, of a medically stable elderly person, of a patient with clinical depression, and of a patient who is intellectually impaired.
